This widget allows you to add Google Forms to your wiki page.
To insert this widget, use the following code:

Parameters
- formkey - a parameter taken from the URL of the form. To find it, choose "Embed" under "More Actions."
- width and height - these can be omitted, but it is better to get them from the embed code generated by Google when you choose "Embed" under "More Actions."
