    Hello,
I am 33-year-old male. First of I would like to thank you for creating this forum. I have been following all the posts regarding TMS for the past 1 week and I think I might be a candidate for TMS but I am having a hard time analyzing. Below is my story.

In the first week of December 2023, I had severe flu and to get rid of that, my family doctor prescribed me a Tamiflu for 10 days. I got better day by day but at the end of the treatment, I started having a pinching-like feeling and heaviness in my groin while urinating. I went to the family doctor again and he diagnosed UTI immediately without any tests and prescribed me Cipro for a week. My symptoms didn't go away so I went to the family doctor once again and he ordered urine tests, STD tests, and pelvic ultrasound. All reports came back normal. Then doctor told me that I had Prostatitis continued Cipro and introduced an alpha-blocker. Due to the side effects of alpha-blockers, doctors referred me to the urologist and stopped all my medications. I got an appointment in March but somehow my symptoms reduced on their own. But I continued with good hydration and cranberry juice.

But after a month, the symptoms returned. Then my family doctor ordered a CT scan but everything came normal. In the meantime, I took nitrofurantoin 100mg and alpha blockers for 2 weeks. But I didn't see any difference. When I visited a urologist in March, he prescribed me a heavy dose of cephalexin and alpha blockers for a month. My symptoms are still on and off. In the meantime, I visited another urologist who told me that I may be suffering from depression there was nothing wrong with my urine system. I got so much confused. I am still taking medicines prescribed by my first urologist.

I have seen at least 8 doctors in the last 3 months. I have been through 6 urine tests, 2 blood tests, an ultrasound, a PSA, and a CT scan. Nothing came out. I still have symptoms like urinary pain, urinary burning, pain in the groin etc. The symptoms continue after urination too. One more thing I would like to mention is I have been through a lot of stressful events in 2023. That stress was so much that I would say it was the highest stress in my whole life. I need help figuring out if this is an actual issue or if it is all in my head.

    Thank you..

    Hello:
In a fairly recent video I saw, Dr. Schecter, a TMS Dr. in California states that almost 100% of chronic pelvic pain patients he has seen, - those with urinary issues, unexplained pain and symptoms have TMS.
Here is the PPD association page on chronic pelvic pain: https://ppdassociation.org/pelvic-pain (Pelvic Pain — Psychophysiologic Disorders Association)

Often, current stressors mirror childhood stressors but the way we dealt with them as children no longer serves us as adults. We don’t express our emotions about what is going on in our lives - as Dr. Sarno notes, especially but not limited to deep rage, sorrow etc. and it shows up in our nervous system and body.
Reading a book by Dr. Sarno (even if your symptoms differ, it’s all the same TMS mechanism) will help you further understand what’s going on and how to address your symptoms.

Thank you for reply. Right now I am having symptoms like urinary burning and pain. The burning starts out of nowhere and I am feeling depressed. I constantly feel on the edge and when the burning or pain starts, I can't concentrate on anything. I am really looking for a help how to cope up with this. One thing is when the burning starts, it burns at the top of my penis, anus and both feet and legs. I have no idea what this connection is. This is so frustrating.

Fear and frustration fuel the symptoms. I know you are physically uncomfortable, but you must soften to both it and the mental/emotional uncomfortable too, even just a little bit. Being frustrated that you can’t fix your symptoms right now is a form of being really hard and harsh and critical of yourself. Taking the focus off of every sensation and trying to stop being so descriptive in your mind and noticing how obsessive that is (without judgement) can help you accept that there is a lot that is playing out, emotionally here. Remind yourself that this is temporary, the depression is simply another symptom and can also get much better. You will be able to get back to life again, you just need to learn how to approach things in your inner life a little differently.
Start with Sarno, the sooner the better. Education about chronic pain is the first step.

Thank you for your prompt reply. I have one question. After reading my initial post, do you think I have tested enough to rule out any structural or bacterial issue? Because that is the first thing to start TMS journey. I am constantly thinking of missing something. Doctor told me that I don't need a cystoscopy. That is the only thing I haven't done.

We aren't doctors, and unfortunately can't answer this question. You have to ask yourself if you feel safe enough to try an emotional approach for a while. Everyone here who has been successful in reducing symptoms has made this decision for themselves.

    Questions to ask yourself:
    1. What benefit have I received from all of these exams?
    2. What do I have to lose at this point by trying something completely different?

Also, at least one doctor was willing to think "outside the box" (as we say in English) :
I would advise exploring this. What we do know and what we can tell you is that many people here have discovered the connection between their urinary and pelvic pain, and their emotional condition.

Thank you for your reply. I have one last question. How can I be sure if this is TMS? Are there any traits? For me it is past three months urinary burning. It gets replaced by groin pain and penis pain sometimes. Sometimes it gets replaced by heavy feeling down there. It then again gets replaced by urinary burning. This is a pattern. But my mind is so much occupied in this that I can clearly sense the symptoms are changing. I am trying to divert but I am failing everytime.

Yes, the symptoms are not always physical. That’s why reading a book by Dr. Sarno is essential. He explains personality types and other factors that people with TMS often have in common. The link I posted above is also helpful in determining the possibility it’s TMS. The first step is self-education. As @JanAtheCPA mentions, it takes willingness to open your mind. One characteristic of TMS folks is the fact they often see things very black and white, and have become rigid in thought. It’s a self-protective way to think.

    You can't.

You also can't be sure of anything all these doctors have done to TRY (and fail) to treat something they can't figure out.

You will need to embrace uncertainty. As @Cactusflower said, clinging to black & white thinking is what leads to frustration, and always blocks healing.
Yes. Read The Mindbody Prescription by Dr John Sarno, MD. Many of your basic questions will be answered.
